Your database contains reports Patients, rptVisits1, rptVisits2 and Visits. What exactly do you want to do with which report? Please provide clear, relevant and sufficient information.

I want to open a form with the interview information based on the input soc sec no and info from the most recent visit from "qryVisits" and other infor from tblPatients. I want to then be able to add new info to the interview and save a new record with the present date.
    Thanks for the help.

Your question is confusing - you appear to be using the term "report" in two different senses: as an Access report and as an overview of data.
Your database is also very confusing, with two Visits tables.
Please post a more consistent database and try to explain what you want in a less ambiguous way.

    Re: Open vivit form (2003)

Patients and their soc sec no and other info is stored in the exist "tblPatients" I want to be able to input a soc sec no of a patient into a new form "frmRe-Eval". This form would draw info of an existing patient , if one exists, from the "tblPatients" if none exists it would request that a new patient be entered in another form that I already have.. Information from the interview field in exist "qry visits" of the most recent visit of this patient would also be displyed in "frmRe-Eval". The soc sec no and the interview would be displayed in a new form. This form would then let me add new infor to the interview field. After info is added to the interview field in "frmRe-Eval" it would be saved with the present date.
Hope this clears up what I am trying to do.
    Thanks for the help.

The only table in this version of the database is tblPatients. The query qryVisits doesn't work since there are no Visits and Current Pain tables. <img src=/S/shrug.gif border=0 alt=shrug width=39 height=15>

In my haste I removed to many tables. They are now in the attached file.
    Thanks again.

The query qryVisits doesn't work, and the form frm New Patient doesn't work. Could you please take a bit of time to check everything before posting yet another version? Thank you.

Does the attached version do what you want? I used a main form for the patient, and a subform for the visits. There is a button on the main form to duplicate the data from the last vist to a new record with the current date.
