    Help / Ideas for form template (2003)

    All,

    I am trying to produce a form which can be viewed by colleagues at work and amended.
    All they will have to do is to put a tick in the box if action is required or not. This is to prevent being sent emails from lots of people to inform me no action is required.
    I think a email template is best but am unsure as to how people would know others have viewed it and therefore put a tick in a the radio box on the form.

    Any ideas / help would be appreciated.

    If you need everyone to be able to view the same information, then a web page linked to some form of data store is probably the simplest thing.
