seeking survey application

I am looking for an application - even web based - that allows me to create surveys with various type questions and comments. Then provides the ability to enter survey responses and produce various reports of the results. We currently create surveys in MS Word and tabulate the results in MS Excel. It works but is not very efficient.

The type survey we do is typically 12 questions and is distributed to an audience during a presentation of some subject of community interest; then the surveys are collected at the end of the program. Questions are like... how did you hear about this program?, your age range? how did you like the speaker?, was the length of the program appropriate? what did you like most/least? other comments?,

Since this is for a non profit organization, low cost (free?) is important.

You can find several free web survey applications using Google, for example SimpleWebSurvey.com Web Survey Software. I don't know how good they are.

We (in the UK) keep on getting asked to complete surveys 'powered by' Survey Monkey. The pricing seems to include a option...

One of the most popular that I know of is: Survey Monkey. Perhaps it will meet your needs.

I also see a lot of use of Zoomerang. Note that the free version does not let you download to Excel: Survey Software Product and Feature Comparison Chart | Zoomerang.

    Re: seeking survey application

Thank you all for your input. I looked at survey monky and it does not meet our needs. I will continue to look at free apps. I am mostly interested in using some app to tabulate and analyse the results more than actually create the survey.
