Fill in the blanks document (Word 2000)

I would like to create a form for a user to fill out like the FAX templates provided with word. Could any one direct me to where I can learn more about this?

    Thanx in advance..

This is a special instance of the MacroButton field. You may want to take a look at the Checkbox Template downloadable from my download page (addbalance.com/word/download/). This is a tutorial and an "Add-In" that can make adding one kind of dynamic checkbox to your documents a lot easier.

You may also want to take a look at my notes on macrobutton field.

You may also want to follow the links at and check out the article How to add pop-up lists to any Word document so you can click your way through changes in seconds (mvps.org/word/FAQs/TblsFldsFms/AutoTextList.htm).

    Hope this helps.
