I am creating a form in Excel 2003. I want to rate questions from 0-5 where each check box has a rating. In the next column I want to display the rating that was checked so that I can total the questiones ratings. What is the easiest way to get this done?

You need to use radio buttons instead of check boxes, and you need to place each group of radio buttons within its own group box.
The linked cell will contain the index of the selected radio button.

Right-click any of the radio buttons.
Select Format Control from the popup menu.
Click in the 'Cell link' box.
Either type the address of the cell or use the mouse to point to the cell.
    Click OK.
This will hold for all radio buttons within the same group box.
