I need a formula that calculates if one column has Attempted suicide and another column also has SDMI that it will count the number of times that occurs. See spreadsheet. The formula is on the Summary Report Table sheet in B5 and the data it is calculating is on the CI Data Entry Sheet.

If you want to specify your criteria you can use:
    =SUMPRODUCT(('CI Data Entry'!$V$3:$V$100="Attempted Suicide")*('CI Data Entry'!$M$3:$M$100="SDMI")*1)

If you want to use column and row headers, which reduces the amount of typing required, you could use :
    =SUMPRODUCT(('CI Data Entry'!$V$3:$V$100='Summary Report Tables'!$A33)*('CI Data Entry'!$M$3:$M$100='Summary Report Tables'!B$4)*1)

The thing to keep in mind is that your column and row headers have to be an match to the data you are calculating against. For example, you would have to remove the 's' from Suicides for this method to work.
