This is hard to answer without referencing particular web sites.
On the first question, some pages contain a redirect. If you go back to that page, it will again redirect you forward to where you just were. To get past it, use the drop-down next to the back/forward buttons to skip past the redirect page. (I'm looking at IE7, but I assume they haven't removed this drop-down in IE8.)
Another possible reason for being unable to leave a page is a script that cancels your navigation. Legitimate sites generally don't use such a script.
