    Have a look at Nuance PDF Converter Professional (www.nuance.com), which is a vast improvement on Adobe PDF products. I've been using it for years and there's virtually nothing it can't do, including creating fillable forms with a rich set of features.

    Update: Checked out the site in detail. You're right: it looks very feature rich! Am currently exploring having the form coded directly into our web page to keep the flow as seamless as possible, but don't yet know costs and time frame. Will keep Nuance in mind if we end up using more of a converter. Afraid this whole fillable forms on our web site is still confusing for me! Hard to know best technical fix to meet our needs.
