Thumbs up Automating an Evaluation Form

I have to create an evaluation form where an employee is evaluated on a scale from 1-4 where one is unsatisfactory. 2 is satisfactory. 3 is Above Average, and 4 is Outstanding. My problem is that the document must be protected. Is there a way to populate a number into a table by having a supervisor click a checkbox next to the evaluation level?

They wanted to go with Word. I guess if I can make it look good, I can create it in Excel. It would definitely make it easier. Thanks for the reply, too.

The layout may arguably be easier to do in Word, but to have a number populate in a Word form based on clicking a checkbox, you'll need to use VBA code. The same functionality can be done in Excel without code, using formulas.

    Thanks Gary

I agree. VBA code is not my forte. If they don't want to create it in Excel, I think they should hire a VBA developer. Thanks for your help, Gary.
