Mail Merge Excel Data to a Word Table. Every third row shifts to right.

I read the tutorial which is advanced for my level. I am very familiar with merging Excel into a Word doc; however, I know nothing of macros, coding, formatting, etc. My project requires that I insert a table containing 4 columns into a Word document. My data source is in Excel. The First two rows of the table merge fine. The third row skipped the first column of data (left it blank) and shifted one column to the right. In other words the information that should have appeared in the first column is in the second column and all else has also shifted. Beginning with the fifth row it happens again shifting right by one more column again. Seems to be a pattern. When I toogle back and forth in each cell of the table the field code seems correct. How do I solve this problem?

Can you attach a sample of the data and doc?
