Question Mail Merge problem - list sorted by category in Word

I have three fields I need to fill so that they look like this:
    A / B / C
Payment Date / Anticipated Payment Amount / Anticipated Payment at this time

    9/2013 / 100 / 100
    9/2014 / 100 / 0

    my data looks like this:
    (1) / 2) / (3) / (4)
Customer Number / Expected Amount / Unique Amount Due This Bill / Expected Date Sql
    123 / $12,500 / $0 / 09/01/14
    123 / $12,500 / $100 / 09/01/13

I've tried using the mergeseq equation on http://support.microsoft.com/?kbid=294686 but I havent' been able to get it to work correctly. Please help?

    the / = column separation
