A document was created and saved and the Table of Contents incorrectly listed page 2 for every item in the TOC.

A change was made to the document (with "revisions" on) and the document was accessed by a second party. The party used the "find revisions" feature to review the changes, noting the above error as he passed by the TOC. After he reviewed all changes, he was asked by Word if he wanted to review the document again. For some reason he clicked "yes" and the TOC had been updated!

Can you tell us what triggered the update, and also why was the TOC not correct to begin with?
