breakout stacked data to single rows

Best to look at the attachment while reading this.

The real data file has several thousand rows. The listings are medical professionals and detailed data.
The breakdown is by PA County which is in the A column.

Some of the professionals have 2 or 3 rows of supporting data.

I want ONE row per medical professional where the columns are: county, full name, specialty, affiliation (if there is one, there are 3 rows; otherwise 2), street address, city, state, zip, telephone.

I haven't figured out my algorithm(s) yet to handle this and thought if someone had done this sort of thing before they'd share the process.

    Thanks.
