Can a UserForm be made into a Class? (XL97 SR-2)

I've put together a settings form with a number of controls (some of them having the ControlSource property) that I'd like to be able to reuse, with start date, interval, and count of intervals for a chart. The book has several charts that need these settings, but rather than duplicate the userform and all its code, I'd prefer to learn how to make a Class of it and call a new instance each time the form is needed. Can this be done in VBA?

It looks as if my question was substantially answered in Wasim's thread below. But how would I go about associating the correct ControlSources when the instance of the form loads?
