Simple form (Word 2002)

I am trying to create a simple fill in form for work. I have went to the MS website and tried to do what they suggest, but my form just does not work out right. I just want to create a simple fill in form. I want to collect information for general things at work. When I have tried to create my forms and save them I am unable to fill in the form or there is a big grey box to fill in. I have included an example of the type of form I want to create and how I want it to look. Am I useing the wrong program? <img src=/S/broke.gif border=0 alt=broke width=31 height=23>

The text boxes in your screenshot seem to be from the VBA controls toolset, but the toolbar is the Forms toolbar.

With the Forms toolbar you create form fields; they usually look gray. You must lock a document (or the section/sections with form fields) for forms in order to be able to enter something in the form fields. The last button on the Forms toolbar locks/unlocks the document. You can also use Tools/Protect document
