    template (XP)

    I would like to create a template for a form that we want to use electronically. I followed the directions from the office assistant but it doesn't work like I want it to. I created the form in word, used Save As: Document Template but when I try to use the form, it moves the lines that I created to type on. For example, I have Patient Name______________. When you type in the name, it looks like: Patient Name:___Doe, John_______. I don't want all my lines moving around or the user to have to delete spaces. I want the user to be able to just type on the line.
