Outlook Form Fields - Missing Data (Outlook XP/2002)

Several years ago a simple questionnaire form was created in Outlook that was sent to a Public Folder. Basically, it is a couple of text fields with a series of yes/no radio buttons. Last December, I went in and updated the form as the powers that be wanted a new question added between #3 and #4. Low and behold, we find today that all of the forms in the Public Folder prior to that change no longer show any data for any of the questions (i.e. a yes no choice). The reason this came up is someone needs to access some info from last year. I understand that the custom fields in this Outlook are tied to an information store on Exchange. What I need is some help to determine if all the information prior to the change has been lost or not. Thanks in advance for any information/direction to go on this.

You might try restoring the form to its original format - if that's possible. The data then might re-emerge. HTH
