Small squares print can't delete in column (Word 2002)

We have a document that is in 2 columns, (created as a table) the far right column is a 2" column with the word Notes at the top to allow users to take notes while in training. This document was originally created in Word 2000 and now we are tweaking it in Word 2002. A small square has appeared, it can not be deleted nor selected, it shows up when printed, however, if we choose the word Notes and change it to white text, the square becomes white and does not print. But you can not go back and double click on Notes and choose black text because the square shows back up. When you delete the word Notes, the square still stays there. It is not on any of the right columns, except it is on the last page that is a single column. Any clues my brainy friends?

If you still have a copy of the problem document, you might remove sensitive information and post it here, so that Loungers can examine it.
