    Um, your post seems to be lacking some detail.
    But here goes with one thing I noticed. You are missing a </form> tag.
    See if that helps, and try posting some more detail in your question. You might need to 'preview' the post first to check that its all showing.

    Oh, and the <form> tag is missing an action= attribute.
