I haven't been able to find a way to add an attachment or item to a recieved message item unless I send it back to myself (I sometimes do that). AFAIK it's only possible to insert a file attachment, or insert another message or a note or task or such via Insert | Item, to messages that are being sent..

However, you can edit the messsage (Edit | Edit Message) and apply fonts or marks that indicate that you made a note. Or you could drag the message to a Note Item, and make notes about the message in the Note body.

If you mean a note as in an Outlook Note....

Open the email you want to add it to, and select Edit > Edit Message
Create the Note and close/save it.
Right_click on the note and select Forward... (a new email opens with the note as an attachment)
Right_click on the attachment and select Copy
Select the original email again, Right_click anywhere in the body and select Paste - the note appears as an attachment.
Save the original, and delete the new one with the surplus note in it.

Kludgy, but it works. On the other hand, John's suggestions above take a fraction of the time....
