We have a COM addin that adds menus and we then remove the menus and we always set the normal.dot template to true so that it will not be saved. We believe another commerical application that also adds menus is not setting the normal.dot template to true and because of that users of that other application are getting a corrupted normal.dot when our COM addin is installed. We what to see by looking in normal.dot what is being modified or added.

As an experiment, you could move or rename Normal.dot, so that you start out without one. If a setting that is stored in Normal.dot is changed, a new instance will be created when you quit Word. You could then open Normal.dot and see if a menu has changed from the default.
