    Favourites in IE

For a long time I have been saying (and believing) that the Title tag is used by browsers for setting Favourites and Bookmarks; and that IE is just variable from version to version in using the information in the title tag or the file name to label the favourite.
However, today working with IE 5 I notice that in fact, some pages IE uses the title tag information and for others uses the file name. And I can't figure out what the difference is. Has anyone else run into this? Is there an attribute or something else I can use to *force* <img src=/S/smash.gif border=0 alt=smash width=30 height=26> IE into always using the title?

Is there a title in the header of the these pages that use the file name?
