    I would use a lookup table in any case:

    <table border=1><td>ID</td><td>Source</td><td align=right>1</td><td>Friend</td><td align=right>2</td><td>School</td><td align=right>3</td><td>Doctor</td><td align=right>...</td><td>...</td></table>
    If there is only one source per application, you can use a number field (Long Integer) to store the source ID, and a combo box bound to this field on the data entry form with the lookup table as row source. If there can be multiple sources for one application, you have a many-to-many relationship, so you would need an intermediate table to store the application-source combinations.
