There is no form, nor any VBA code in the workbook you posted. <img src=/S/confused.gif border=0 alt=confused width=15 height=20>

    making a form (excel 2000)

I am making a spreadsheet which i am trying to automate.
Here I have a list of different types of sand on sheet1.

I am trying to make a form which will fill out my proppant schedule.

There is a button called Proppant which will open up a form which will ask
    Proppant1
    Amount:
    Mesh:

    Proppant2
    Amount:
    Mesh

    Proppant3
    Amount:
    Mesh:

    Proppant4
    Amount:
    Mesh:

click ok and the cells in column c and d will be filled out

right now I can make a drop down list but want to see if I can automate this task
    any help would be great.

    Thanks ahead.

I would use combo boxes instead of list boxes. Set the RowSource of the combo boxes to Proppant!A2:A43.
Set the ControlSource of the text boxes and of the combo boxes to the cell they should be linked to.

    See attached version.
