    You need to save the form as a form/template and when you want to use it, select File/New chose form. Should work just fine. I use it all the time.
    Be sure to not use Word as email editor when you create the form.

    I did everything it said in the Help file to create my form/template. When I send it to someone, they just receive a blank email. Shouldn't I be able to fill in the information into my form and then email it to someone and they can see the info I filled in? I'm attaching the form I made, if that helps at all.

    (I forgot to attach it in my previous attempt at posting.)

    I use templates and have no problems with them. However, when I tried your, I had the same result you had.
    Look at the attached, modified form and see if it works for you.

    I know, that was an accident. Sorry! My problem is still the same. I used the instructions sent and created my template and it still doesn't work. I can open it and see all the fields and everything I created, but when I email it, it's blank. Could it have anything to do with settings on our email server?

    Try looking in Oulook help, table of contents, forms. Under troubleshooting forms there is one item that says:
    new items are not using my custom form.
    The answer is that you need to set the default form for the folder of your custom form.
    I do not know if that will help.

    I finally found the answer!!

    Disable the Separate Read Page option for that form so that the same page is viewed when in Compose or Read mode. To do this when the form is in design mode, on the Form menu, click to clear the Separate Read Layout check box.
