Custom Form (2003)

A custom form was created from the IPM.Appointment template. This form was published and placed in the Outlook Organizational Forms Library.

What I will like to do, is to place this form as a menu item or assign it to a button on the toolbar. How can I call a user defined custom template in a module, creating a macro which will open the form?
