Form is not cleared (Access 2000)

When i open a report from a control on a form, the form is not cleared and when i close the report the form shows the previous choice. The two alternatives, either closing the form on openig the report or clsoing the form in the OnClose event of the reprot also do not give results. What may be the reason ? .I am enclosing a very chopped up example of my database, just to show that the form stays put on the place where it was before opening the report.

It is not clear what your form is intended to do. It has a subform that is unbound, it has a text box City that is bound to a field that does not exist.
What do you expect / want to be cleared?

Thank you for the reply.I just wanted to show the problem, but i have cut most of the functions and controls, for example the subform is empty, and therefore maybe the problem is not clear. If you are not statisfied then i will need to send a fuller version. What i want is the following. If you choose for example the client swww and click the report, the report opens, but when you close it the form remains with the client swww, while i want everything to be cleared. In my fuller version, when i choose a client to send an offer, other details also appear and they all stay put, when the report is closed. If you help me build the form and the report so that when i close the report, the form is clean and ready for the new client..

The combo box ClientID is bound to the ClientID field in Query1 and through that, to the ClientID field in tblOffers. So if you select a client from the combo box, you are in fact modifying a record in tblOffers.
If you want to use the combo box merely to select a client to be displayed on the report, the combo box should be unbound, i.e. its record source should be empty.
