    The only "attachment" I can see is the graphic showing the intended appearance of your form.

    What problem do you have when trying to create this form? What specific help do you need?

    Here is a very simple example of your form done in Word. I simply placed the graphic behind the text and used line spacing and tabs to place the check boxes in the correct places.

    I zipped the document as it was too big to post.

    Thanks, this form is for student, is there a way of getting test results after submitting the test. i will attach another example on how it should look like.

    Hi everyone, i have a problem with creating a form that will give results to student, I tried creating it in Access and Word. I have attached an examples on how it should look like at the end of the test. It should give them a score and specify whether they passed or not.
