    It could be the add-in crashed at one point or an Outlook update thought the add-in was not compatible.

    As far as the 32/64 bit version goes, you probably have the 32-bit version of Office so you need the 32-bit version of the add-in. A 64-bit machine and OS don't determine this. Many Office add-ins have not been updated to support 64-bit so Microsoft still recommends 32-bit Office unless you have specific requirements such as very, very large spreadsheets.
